Scout Registration

This page contains information on how to register to join Pack 45.

Registration Fees for the scouting year are $150 per scout (plus a $25 ONE-TIME registration fee, for new scout if applicable). 
Payment via Venmo @pack45mantua , check or cash or payment plan if needed.

RETURNING SCOUTS

Please download, print, and complete the following forms and provide to your Den Leader:
  • Registration Form (Obtain at Pack Meeting or from Den Leader)
  • BSA Youth Application (PDF) - Only needed if contact information has changed from last year
    • (Print and submit page 5 / Ignore fees entry)
  • BSA Health & Medical Record Forms A&B (PDF)
  • Copy of both sides of Scout's Medical Card
  • Submit payment via Venmo @pack45mantua

NEW SCOUTS
